What you would do
- Implement physical therapy treatment plans directed by the supervising PT
- Deliver therapeutic exercise, gait training, and balance activities
- Monitor and document patient progress and treatment responses
- Communicate with PT, interdisciplinary team, and nursing staff
- Educate patients and families on home exercise and fall prevention
- Participate in care planning, utilization reviews, and MDS documentation
Must have
- Associate's degree from accredited PTA program
- Current unrestricted Colorado PTA licensure or ability to obtain
- Current CPR/BLS certification
- At least one year skilled nursing or post-acute experience
- Experience with geriatric populations and fall prevention
Nice to have
- Familiarity with MDS/PDPM documentation frameworks
