What you would do
- Implement physical therapy treatment plans as directed by supervising PT
- Deliver therapeutic exercises, gait training, balance activities, and modalities
- Monitor and document patient progress and treatment responses
- Communicate with PT, interdisciplinary team, and nursing staff
- Educate patients and families on home exercises and fall prevention
- Participate in care planning, utilization reviews, and MDS documentation
Must have
- Associate's degree from accredited PTA program
- Current unrestricted PTA licensure in Colorado or ability to obtain
- Current CPR/BLS certification
- Strong interpersonal and communication skills with patient-first mindset
Nice to have
- One or more years in skilled nursing or post-acute rehab
- Familiarity with MDS/PDPM documentation frameworks
- Experience with geriatric populations and fall prevention programs
- Knowledge of EMR systems used in skilled nursing facilities
