What you would do

  • Implement physical therapy treatment plans as directed by supervising PT
  • Deliver therapeutic exercises, gait training, balance activities, and modalities
  • Monitor and document patient progress and treatment responses
  • Communicate with PT, interdisciplinary team, and nursing staff
  • Educate patients and families on home exercises and fall prevention
  • Participate in care planning, utilization reviews, and MDS documentation

Must have

  • Associate's degree from accredited PTA program
  • Current unrestricted PTA licensure in Colorado or ability to obtain
  • Current CPR/BLS certification
  • Strong interpersonal and communication skills with patient-first mindset

Nice to have

  • One or more years in skilled nursing or post-acute rehab
  • Familiarity with MDS/PDPM documentation frameworks
  • Experience with geriatric populations and fall prevention programs
  • Knowledge of EMR systems used in skilled nursing facilities
