What you would do

  • Manage patient census to close clinical and non-clinical care gaps
  • Identify and coordinate community resources for patient benefit
  • Provide patient education on chronic condition management
  • Perform medication management including reconciliation and refill coordination
  • Ensure timely delivery of services like home health and DME
  • Resolve patient questions and foster open dialogue

Must have

  • Active New York LPN license
  • At least 3 years direct patient-facing experience
  • Technical aptitude with Microsoft Office Suite
  • Excellent written and verbal communication skills
  • Active high-speed Wi-Fi connection
  • Home office or HIPAA-compliant workspace
  • Ability to sit for long periods
  • Ability to speak clearly and listen attentively by phone
  • Ability to take thorough notes in real-time on calls
