What you would do

  • Manage patient census to close care gaps
  • Coordinate community resources for patient care
  • Educate patients on chronic condition management
  • Perform medication management and reconciliation
  • Ensure timely delivery of home health and DME services
  • Resolve patient questions and foster open dialogue

Must have

  • Active New York LPN license
  • At least 3 years direct patient-facing experience
  • Proficiency in Microsoft Office Suite
  • Excellent written and verbal communication skills
  • High-speed Wi-Fi connection
  • Home office or HIPAA-compliant workspace
  • Ability to sit for long periods
  • Ability to speak clearly and listen attentively
  • Ability to take thorough notes in real-time
  • Self-motivated and detail-oriented
  • Thrives in fast-paced environment
  • Experience coordinating chronic care needs
