What you would do

  • Manage patient census to close gaps in care
  • Coordinate community resources for patients
  • Provide education on chronic condition management
  • Perform medication management and reconciliation
  • Ensure timely delivery of home health and DME services
  • Resolve patient questions and understand needs

Must have

  • Active New York LPN license
  • At least 3 years direct patient-facing experience
  • Technical aptitude with Microsoft Office Suite
  • Excellent written and verbal communication skills
  • Active high-speed Wi-Fi
  • Home office or HIPAA-compliant workspace

Nice to have

  • Epic experience

  • Additional state LPN licensures
