The role

CareHarmony is hiring a remote Licensed Practical Nurse (LPN) to work as a care coordinator, managing patient care and coordinating resources for chronic care management. The role requires a Minnesota LPN license and at least 3 years of direct patient-facing experience. This is a full-time, Monday through Friday position with a consistent schedule and no weekends.

Must have

  • Active Minnesota LPN license

  • Home office or HIPAA-compliant workspace
