What you would do
- Handle assigned inbound and outbound calls within scope of practice
- Consult with on-call providers and other clinicians as needed
- Coordinate telehealth meetings between participants and clinicians
- Ensure timely care delivery and resolve basic issues
- Escalate issues to management when necessary
- Troubleshoot logistical care issues like transportation and medication delivery
Must have
- Graduate of an accredited school of nursing
- Unencumbered RN license
- Nursing knowledge to treat frail elderly participants
- Ability to manage complex clinical situations
- Highly motivated and self-directed
- Able to execute tasks in a quickly changing environment
- Sound decision-making in emergency situations
- Excellent clinical, organizational, and communication skills
- Able to work assigned shifts including nights and weekends
