About WelbeHealth
WelbeHealth runs a PACE program that helps seniors stay in their homes by providing comprehensive medical care and community-based services.
What you would do
- Conduct home health nursing assessments to determine equipment needs and participant goals
- Participate in interdisciplinary team meetings to develop care plans
- Deliver and document nursing interventions per care plans, including medication administration and tests
- Instruct participants, families, and caregivers on disease process and self-care techniques
- Track and monitor home health scheduling with the Home Health Manager
- Collaborate with physicians and IDT for smooth care transitions between settings
Must have
- Graduate of an accredited school of nursing
- Unencumbered RN license required
- Minimum one year nursing experience with frail or elderly population
- Reliable means of transportation
- Obtain CPR and First Aid certification within 180 days of hire
Nice to have
- Home health agency experience preferred
- CPR and First Aid certification upon hire preferred
