What you would do
- Provide psychiatric consultation, evaluation, counseling, and treatment for participants
- Perform comprehensive psychiatric assessments in-person or via telehealth with physician collaboration
- Collaborate with interdisciplinary team to develop comprehensive care plans
- Assist with discharge planning from acute or long-term care settings
- Refer participants to psychiatrists as needed in consultation with physicians
- Oversee nursing home participant care with regular visits as required
Must have
- Master's degree in Nursing or Physician Assistant with psychiatric/mental health emphasis
- Certified as Psychiatric Mental Health Nurse Practitioner or Physician Assistant
- Active state license without restrictions
- State CDS Certification and DEA License
- NPI license
- Reliable means of transportation
- Minimum two years experience working with frail or elderly
- Obtain CPR and First Aid certification within 180 days of hire
