About WelbeHealth

What you would do
- Perform comprehensive in-person assessments for participants
- Lead and partner with interdisciplinary team to develop care plans
- Coordinate discharge for acute, short-term, and long-term placements
- Provide holistic care with an average of 8 patients per day
- Conduct home visits as part of regular duties
- Participate in rotating on-call schedule by phone
Must have
- Completion of ACGME-accredited residency program
- Master's or doctorate degree
- Active state physician license without restrictions
- State CDS certification and DEA registration
- Eligibility for Medicare and Medi-Cal enrollment
- Obtain CPR and First Aid certification within 180 days
- Specialization in Internal, Family, Emergency Medicine or Geriatrics
