About WelbeHealth
WelbeHealth provides community-based care to vulnerable seniors through a value-based PACE program.
What you would do
- Perform comprehensive in-person assessments for participants
- Lead and partner with interdisciplinary team to develop care plans
- Coordinate discharge for acute, short-term, and long-term placements
- Provide care to an average of 8 patients per day
- Conduct home visits as part of care delivery
- Focus on comprehensive care without hospital rounds
Must have
- Completed residency program accredited by ACGME
- Master's or doctorate degree
- Active state physician license without restrictions
- State CDS certification and DEA registration
- Eligible to enroll for Medicare and Medi-Cal services
- Obtain CPR and First Aid certification within 180 days
- Specialization in Internal, Family, Emergency Medicine or Geriatrics

- Board certification
