What you would do
- Supervise and instruct home care assistants and aides in participants' homes
- Educate participants, families, and caregivers on disease and self-care
- Conduct home safety assessments as assigned
- Deliver nursing interventions, administer medications, and perform ordered tests
- Support smooth care transitions between care settings
- Work varied schedule including evenings, nights, and weekends
Must have
- Graduate of an accredited school of nursing
- Unencumbered LVN license in the state of practice
- Reliable means of transportation
- Nursing knowledge to treat frail elderly participants
- Ability to manage complex clinical situations
- Obtain CPR and First Aid certification within 180 days of hire
Nice to have
- CPR and First Aid certification upon hire
