The role
The Case Manager, RN directly guides participant care as a crucial voice on the interdisciplinary team (IDT). Reporting to clinical leadership, the role delivers and oversees all aspects of care, including conducting required PACE assessments, developing and following personalized participant care plans, and supporting transitions of care coordination. The role offers a consistent Monday through Friday schedule with no weekends or on-call duties.

What you would do
- Conduct clinic nursing assessments to determine participant needs, preferences, and goals
- Participate in IDT meetings to develop participant care plans
- Deliver and document nursing interventions as agreed upon in participants' care plans
- Maintain a safe environment, respond to physician orders, and administer medications
- Collaborate with the PACE Physician and other IDT members to coordinate participant care
- Communicate with participants and their families to revise care plans
Must have
- Graduate of an accredited school of nursing
- Unencumbered California RN License
- Current Basic Life Support (BLS)/First Aid Certification required
- Must obtain CPR and First Aid certification within 180 days of hire
- Maintain CPR and First Aid certification thereafter
Nice to have
- Bachelor of Science (BSN) preferred
- Two years of nursing experience in a clinical setting with a frail or elderly population preferred
- CPR and First Aid certification upon hire preferred
- Experience working with a frail and elderly population
