What you would do
- Perform initial and ongoing comprehensive patient assessments
- Develop, implement, and revise the plan of care
- Educate patients and families on disease and end-of-life care
- Coordinate care with the interdisciplinary group and families
- Provide clinical direction to hospice aides and LPNs
- Document progress and ensure quality of care
Must have
- Graduate from accredited nursing school
- Current state RN license
- Current driver's license
- Two years clinical care RN experience
- Reliable transportation and auto liability insurance
- Ability to read, write, and comprehend English
- Ability to lift up to 50 pounds
- Ability to work flexible schedule and travel locally
Nice to have
- Hospice experience
- Specialized skills in palliation and end-of-life care
Experience and education
- 2+ years of relevant experience
- Associates degree or equivalent experience
