What you would do
- Administer nursing care to patients in their homes
- Coordinate care with interdisciplinary team, families, and referring agencies
- Perform comprehensive initial and ongoing patient assessments
- Develop and document patient plans of care
- Educate patients and families on disease process and self-care
- Provide clinical direction to hospice aides and LPNs
Must have
- Graduate from accredited nursing school
- Current state RN license
- Current driver's license
- Two years RN experience in clinical setting
- Nursing skills per generally accepted standards
- Good interpersonal skills
- Must read, write, and comprehend English
- Reliable transportation and valid auto liability insurance
- Ability to work flexible schedule and travel locally
- Ability to lift up to 50 lbs and move patients
Nice to have
- Hospice experience
- Specialized skills in palliation and end-of-life care
Experience and education
- 2+ years of relevant experience
- Associates degree or equivalent experience
