What you would do
- Conduct initial and ongoing comprehensive patient assessments
- Develop, implement, and revise the plan of care
- Educate patients and families on disease and end-of-life care
- Coordinate care with the interdisciplinary group and team
- Provide clinical direction to hospice aides and LPNs
- Prepare clinical and progress notes documenting patient progress
Must have
- Graduate from accredited Diploma, Associate, or Baccalaureate nursing school
- Current state license as a Registered Nurse
- Current driver's license
- Two years experience as a registered nurse in clinical care
- Nursing skills per generally accepted standards of practice
- Good interpersonal skills
- Must read, write, and comprehend English
- Reliable transportation and valid auto liability insurance
- Ability to work flexible schedule and travel locally
- Ability to lift up to 50 lbs and move patients
- Prolonged standing and walking required
Nice to have
- Hospice experience preferred
- Specialized skills in palliation and end-of-life care preferred
