The role
This is a mobile, home-based Physical Therapist position serving patients in the San Fernando region. The role involves traveling to patient homes to perform evaluations, develop care plans, provide therapeutic interventions, and educate patients and caregivers. It is a full-time 1099 independent contractor position with a 4/10 work schedule.

What you would do
- Travel to patient homes in the assigned service area
- Perform comprehensive physical therapy evaluations in home settings
- Assess strength, range of motion, balance, gait, and functional limitations
- Evaluate home environments for safety and fall risks
- Develop individualized plans of care based on evaluations and physician orders
- Establish measurable short-term and long-term functional goals
Must have
- Current and valid California Physical Therapist license
- Graduate of an accredited Physical Therapy program
- Strong knowledge of anatomy, physiology, kinesiology, and biomechanics
- Strong clinical reasoning and patient assessment skills
- Ability to independently perform comprehensive PT evaluations
- Strong knowledge of therapeutic exercise and gait training
- Excellent patient assessment and pain management skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage a mobile caseload
- Punctual, dependable, organized, and professional
- Comfortable entering and providing care in patients' homes
- Valid driver's license and reliable transportation
Nice to have
- Experience with orthopedic, musculoskeletal, or neurological conditions
- Previous home health, mobile therapy, or community-based experience
- CPR/BLS certification
What you get
- Guaranteed base pay of $1080 per week
- Base compensation at $45 per patient visit for minimum 6 visits daily
- $45 per additional patient visit beyond daily base of 6
- Four 10-hour workdays per week
- Autonomy and flexibility in a mobile role
