What you would do
- Perform skilled nursing visits including medication management and injections
- Conduct client assessments and identify risks and strengths
- Develop comprehensive care plans and recommend care levels
- Coordinate care with team members and attend medical appointments
- Oversee quality of services and manage care coordination
- Conduct home visits and communicate plans of care
Must have
- Active California Registered Nurse (RN) license
- 1+ years paid clinical nursing with older or disabled adults
- Excellent written and verbal communication skills
- Ability to travel throughout San Diego County
- Moderate+ technology proficiency including Google Workspace
- Work independently and manage multiple projects
- Active driver's license and clean driving record
- Current CPR certification
- Pass HCSB background screen, TB test, drug screening
- Ability to lift up to 40 lbs unassisted
Nice to have
- Bachelor's degree in nursing (BSN)
- Working knowledge of physical and mental health care in various settings
- Knowledge of care resources in San Diego County
