What you would do
- Conduct HRSN screenings and eligibility assessments for patients.
- Refer patients to community-based resources and programs.
- Manage closed-loop referrals and coordinate services.
- Assist with benefits enrollment and health system navigation.
- Provide culturally competent health education to patients.
- Maintain accurate documentation of communications and referrals.
Must have
- Associate degree in Human Services or related field.
- One to three years of experience engaging community partners.
- Strong organizational skills with knowledge of Excel.
- Knowledge of data tracking.
- Valid NYS driver's license.
- Ability to travel between sites as needed.
Nice to have
- Minimum of 5+ years related experience in clinical, non-profit, or managed-care environment.
- Extensive knowledge of health equity and social drivers of health.
- Knowledge of social care data.
- Experience working or living in the target community.
- Knowledge of community resources.
