What you would do
- Initiate contact with patients referred for psychosocial support
- Conduct psychosocial assessments and develop care plans
- Provide individual, group, and family counseling including crisis intervention
- Identify strengths, coping skills, and support systems of patients and caregivers
- Facilitate connections to community, governmental, and national resources
- Collaborate with physicians and healthcare team in treatment planning
Must have
- Master's degree in social work
- Minimum two years healthcare social work experience
- Current Tennessee state license with LCSW credentials
- Excellent verbal and written communication skills
- Ability to work autonomously and manage multiple priorities
- Strong organizational skills and interdisciplinary collaboration
- Proficiency in documentation and healthcare technology systems
