What you would do
- Provide physical therapy services in patients' homes
- Evaluate patients' mobility, strength, and functional abilities
- Develop individualized treatment plans to improve independence
- Monitor patient progress and adjust treatment plans
- Educate patients and families on exercises and safety
- Collaborate with nurses, physicians, and other professionals
Must have
- Graduate of accredited Physical Therapy program
- Current Connecticut Physical Therapist license
- At least 1 year of experience as practicing Physical Therapist
- Strong communication and interpersonal skills
- Compassionate approach to patient care
- Ability to work independently and manage priorities
- Organize a flexible patient schedule
- Genuine passion for helping patients achieve mobility
