Clinical Intake Specialist

What this role involves

The Clinical Intake Specialist conducts outreach and intake visits for clients with complex care needs, performing assessments and coordinating with the care team. The role requires an active LVN license or related degree/certificate, at least 2 years of intake experience, and proficiency with health record systems. The position is based in specified states and involves working Monday-Friday 8:30am-5pm Pacific Time.
